Federal Qualified Health Centers (FQHC) are required to periodically demonstrate and document the needs of its target population. The Needs Assessment will include demographic data for your services area that will include: population to primary care physician FTE ratio; percent of population at or below 200% of poverty; percent of uninsured population; proximity to providers who accept Medicaid and/or uninsured patients; and health indicators (e.g., diabetes, hypertension, low birth weight, immunization rates).
